Vision benefits are provided by EyeMed. When you use an EyeMed Insight network provider, the Plan pays more. Vision is a separate election. If you do not elect the vision benefit, you will have no routine eye exam coverage.
The plan will feature a new free annual eye exam at all network provider locations. For 2020, providers will be available through the EyeMed Insight network. Be sure to confirm that your provider participates in the network so you can receive the in-network level of benefits..
Go to Eyemed.com click on Find a Provider (don’t log on if you are a current member). Then, enter your zip code and select Insight from the network drop down to get a list of in-network providers in your area.
Freedom Pass
With the Freedom Pass program, members will receive free frames at LensCrafters and Target Optical. This applies to frames from top leading brands such as Ray-Ban, Coach, Oakley and more. To take advantage of $0 out of pocket cost for frames of your choice, you'll first need to access the offer. For LensCrafters, go to freedompass.eyemed.com and enter EMFP20 to get your in-store offer code. Bring the code to your local LensCrafters to redeem for new frames. For Target Optical, access and print the Freedom Pass document posted to the right under Related Content and Links. Use the in-store code of 755288 at the point of sale.
The plan will offer copay coverage for specialty lenses, such as standard and premium progressives, standard and premium anti-reflective coatings and even photochromic plastic lenses.
